About the Role
We are seeking a detail-oriented and organized Clinical Support Specialist to join our team supporting both Chronic Care Management (CCM) and / or Behavioral Health Integration (BHI) programs. This fully remote role focuses on data entry, documentation, and administrative support, ensuring accurate and efficient management of patient records, care plans, and provider documentation for multiple care programs.
What are CCM & BHI?
- Chronic Care Management (CCM): A structured program designed to provide coordinated care for patients with multiple chronic conditions. Proper documentation and data management are critical to supporting care plans, compliance, and reporting.
- Behavioral Health Integration (BHI): A patient-centered approach that integrates behavioral health services with medical care, improving access, coordination, and support for patients with mental health concerns. Accurate data and documentation are essential for effective care coordination and reporting.
Key Responsibilities
- Enter and update patient information, chronic condition details, behavioral health histories, and care documentation in electronic health record (EHR) systems
- Maintain organized and accurate clinical and administrative records for CCM and BHI programs
- Assist providers with documentation review, care coordination tracking, and record management
- Manage scheduling data, referrals, and follow-up tracking (administrative only)
- Support reporting and quality tracking for CCM and BHI programs
- Ensure data integrity, confidentiality, and compliance with HIPAA standards
- Assist with insurance verification, billing documentation, and claims processing as needed
Qualifications & Requirements
- High school diploma or equivalent required
- Minimum of 1 year of experience in a healthcare support or administrative role with data entry responsibilities
- Excellent attention to detail and organizational skills
- Strong written communication skills
- Proficiency with electronic health records, telehealth platforms, and data management software
- Ability to manage multiple tasks and prioritize efficiently in a remote setting
- Knowledge of chronic care and behavioral health terminology preferred
- Commitment to HIPAA and confidentiality standards
