Position Type: Full-Time | Monday–Friday | Day Shift

Are you passionate about improving the health and well-being of older adults? At Suburban Geriatrics, we're seeking a dedicated Nurse Practitioner to provide comprehensive, on-site primary care services to residents in Assisted Living communities.

This is an opportunity to practice patient-centered medicine in a setting where you can develop lasting relationships with residents, collaborate closely with Assisted Living staff, and help seniors maintain their independence and quality of life.

As an integral part of our interdisciplinary care team, you'll work alongside community leadership, nursing staff, families, and caregivers to deliver proactive, personalized care that keeps residents healthier and reduces unnecessary hospital visits.

• Providing routine primary care and annual wellness visits

Who You Are

• Licensed Nurse Practitioner
• Passionate about caring for older adults and improving their quality of life
• Geriatric or primary care experience is preferred, but new graduates are welcome to apply
• Compassionate, collaborative, and committed to providing exceptional patient-centered care
• Comfortable working independently while partnering closely with Assisted Living teams
